The best cartilage grafts for rhinoplasty are obtained from the nasal septum, inside the nose. This is because the cartilage of the septum is strong and maintains its shape. These two qualities are not found in any other type of cartilage in the body. If the patient has had a septoplasty before, cartilage can be obtained from the ear. This is done through a small incision that is hidden behind the ear. The ear does not change in shape or appearance after obtaining the cartilage. The downside of using ear cartilage in rhinoplasty is that it is not as strong and has an irregular shape.

What Is The Importance Of The Skin In A Rhinoplasty?

The skin and soft tissue that covers the bone and cartilage is an important part of the nose. It is important to understand how the thickness of the skin influences the result of the rhinoplasty. The edges of the cartilage and bones have to be perfectly rounded so that they cannot be seen through the thin skin. I also avoid the use of certain types of grafts in these patients, to avoid any showing through the skin. The contouring of the bone and cartilage framework must be flawless. Any small irregularities will be noticeable once the post-operative swelling has subsided.

Other times I use the septum support to support the nasal tip. I also thin the skin from the inside to help improve definition. The use of structural rhinoplasty is very important in patients with thick skin. The tip of the nose needs to be strengthened in order to combat excess weight from thick skin. If the tip of the nose is not strengthened, the extra weight of the skin tends to cause the tip of the nose to drop. When this happens, the tip loses definition, projection and also rotation. Patients with thick skin can also suffer from prolonged swelling and the formation of scar tissue. Sometimes I use steroid injections to help lower the swelling in my nose.
